Free/Reduced Price Meal applications are available on our school website and in each office.
Student Fee Waiver: Will be sent to those who qualify for free/reduced school meals. 

Kindergarten Enrollment: A child must be 5 on or before September 15, 2026 to enroll in Kindergarten. Birth Certificate and copies of immunization must be on file before a child is enrolled. 

Preschool Enrollment: A child must be 4 on or before September 15, 2026 to enroll in Preschool. Birth Certificate and copies of immunization must be on file before a child is enrolled.

Iowa law requires parents of kindergarten students and all new students entering this school system to provide proof of immunization before a child can be enrolled. For additional information or to schedule immunizations, please call your provider.

*Preschool and Kindergarten students must be up to date on immunizations or provide medical or religious exemption.

*Students entering 7th grade are required to have the meningococcal and tDap immunization BEFORE the first day of school, or religious/medical exemption.

*Students entering 12th grade are required to have the second dose of the meningococcal immunization BEFORE the first day of school, or religious/medical exemption.

Other Health:
*Incoming Freshman: Dental Screening performed by a licensed dentist or dental hygienist. IDPH dental screening form must be completed and returned to the school office or nurse.
(No earlier than 1 year prior and no later than 6 months after enrollment.)

*If your child will be taking medication at school, please have your provider fill out the Long Term Medication form and fax to: 641-622-3604. All prescription medication must be in a bottle from the pharmacy

Release of Student Information: 
All school parents should be aware that the information listed below may be released to the public or military in regard to any individual student of the school district as necessity or desirability arises. Any student, parent or guardian not wanting this information released to the public or military must make an objection in writing to the principal or other person in charge of the school which the student is attending. It is desirable to renew this objection at the beginning of each school year. 

Name, address, telephone listing, date and place of birth, participation in officially recognized activities and sports, weight and height of members of athletic teams, dates of attendance, awards received, the most recent previous school or institution attended by the student and other similar information. 

A designated employee from the district will be responsible in contacting DHS when there is suspicion of child abuse.  DHS will conduct the investigation. 

NCLB Requirement Parents'/Guardians' Right Notification 
Parents/Guardians in the SCSD have the right to learn about the following qualifications of their child's teacher(s): 

  • State licensure requirements for the grade level and content areas taught 

  • The current licensing status, baccalaureate/graduate degree(s), additional certification(s)

  • Qualifications of an instructional paraprofessional who serves your student in a Title 1 Program

  • Parents/guardians may request this information from Central Office by calling 641-622-2025.
